Challenge
A pipeline company was installing a new 35-mile length of pipeline in Michigan to serve as a feeder line into a large terminal as part of an expansion project. The client, a Loss Prevention System (LPS) company, requires its contractors to adhere to the behavior based health and safety protocols; however, the pipeline construction workers were experiencing injuries and property damage during installation of the pipeline.
Challenge
The client, a liquid-petroleum transportation service company, required assistance with disposal of polychlorinated biphenyl (PCB)-containing transformers at three pump stations. These sites were undergoing upgrades and equipment replacement, which required removal of several electric transformers containing PCB oil. A removal plan had been developed based on previous projects that required transformers and drums to be placed in a roll-off for removal.
Challenge
The client, a liquid-petroleum transportation service company, transports refined fuel from Illinois and Indiana to Michigan via its pipeline. The client installed a new control room at its corporate office in Portage, and is expanding its field offices in Niles and Freedom Township, MI. However, the client did not have sufficient safety staff to assign a health and safety inspector at each of the three construction sites.

The client became concerned about their construction contractors’ commitment to the use of behavior-based safety tools when several construction safety issues arose, and determined to take proactive steps before a more significant incident occurred.
